Insurance Information

Dr. Loos is an "In-Network" provider with most of the major medical insurance carriers - just have your ID card handy when you call and we can check your coverage and co-pay before your initial visit. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are check, cash, or any major credit card.

Consultation & Office Visit Fees

Dependent upon validation of insurance coverage and applicable co-payment, our fees are as follows:

Initial Evaluation & Consultation: $75.00

Office Visit: $45.00

**All Co-pays and Fees are due at the time of your visit**
